PPC Bully
PPC Bully uses a slightly different lead page with a wider template, 2 columns and 9 video testimonials. There is a 60 day gurantee and a single large Add to Cart button on the bottom. I personally like this format for a lead page a lot more because it uses more of the real-estate of my 24in monitors.
They have a 3 step process for making money with PayPerClick and what you get is access to their tools which help automate the process of finding products to sell and finding keywords.
The also have a free toolbar that has some lookup capabilities.

Affiliate Launcher
Affiliate Launcher from Andrew X and Steven Lee Jones have an internet affiliate building system. As is typical of these types of sites they use a lead generation page that seems to go on forever. While in the past these types of pages were popular because the idea was someone is less likely to leave if they see just a single page. They are more likely to keep scrolling down so multiple calls to action throughout the long page.
This page also uses multiple headlines every 4 in or so and includes screen caps of clickbank results, as is also quite popular. They have a 6 step system that talks in general terms about the steps involved in doing the system followed by more detailed explimation and another clickbank screen cap.
